Scouting Forms

These forms are provided in a variety of formats, denoted by the following codes:

  • No format indicated (hyperlink) = you may view and print this form from your web browser.
  • PDF = Adobe Acrobat Portable Document Format (PDF). You need a copy of Adobe Acrobat Reader, which you can obtain free on the Adobe website.
  • RTF - Rich Text Format - used by Microsoft Word and some other products.
  • WFW = Microsoft Word for Windows 6/95 or 97 (WFW) format.
  • WP5 = Word Perfect 5.1 for DOS
  • WPD = WordPerfect 6.1 for windows.

Note: Wherever possible, the locally stored forms have been replaced by links to BSA National's own forms site, so as to always have the most up to date forms. Forms from that site are flagged as "link to BSA site"

  • Advancement Form - These forms should be submitted in triplicate - Council, District and Troop copies.

  • District Award of Merit Nomination - link to BSA site - PDF - for nominating someone for this award, the highest a district can give a Scouter. Don't forget to get your nominations in to the district committee in time for the District Recognition Dinner!

  • Flight Permit: PDF - required for orientation or Aviation Merit Badge flights - no flight instruction permitted.

  • Medical Forms
    Class 1 -
    for day camps, hikes, etc. under 72 hours in duration. Not for adults over 40 or high adventure.
    Class 2
    - for campouts, resident camp, longer than 72 hours - youth and adults under 40; not for high adventure - must submit BOTH class 1 and class 2 forms.
    - In PDF format, Class 1 and 2 combined: link to BSA site PDF
    Class 3 - for adults over 40 and all participants in high adventure programs link to BSA site - PDF
    Baden-Powell Council Combined Medical Form - MS Word  PDF - this form has been modified from the BSA form to include NYS requirements. 
  • Meeting Plan Form - WFW troop written form - PDF from National site
  • Merit Badge Counselor Application link to BSA site PDF
    Note: new MB counselors must also submit an Adult registration form, even if they are already registered in some other capacity.
  • Money Earning Permit Application Required before running a unit fundraiser - link to BSA site PDF

  • Scoutmaster Award of Merit Nomination - PDF - for use by a troop in nominating their Scoutmaster for this award, granted by the National Eagle Scout Association (NESA)
  • Tour Permits:
    Local Tour Permit:
    Entire form in one file - link to BSA site (PDF only). For trips under 500 miles, entirely within the USA.
    National Tour Permit: link to BSA site - (PDF only) Used for trips over 500 miles, or outside the USA.
  • Transfer Form - Youth WFW - Adult WFW - Both PDF
  • Troop Resource Survey: PDF - survey your parents for help in your troop's program.
  • Uniform Inspection Sheet - Leaders  PDF
  • Veteran Scouter Award Application - PDF - for applying for Veteran year pins (every 5 years). Note that pins up to 20 years are awarded by Council, 25 years and up are from National. Submit the application to the Council office in any case.
  • World Conservation Award (the panda patch)- Cub PDF - Scout PDF

  • Job Title Quiz - a fun game for your Scouts - how many of these job titles can they define? WFW (oh yes - and the answers WFW)
  • Quartermaster Equipment Form - WFW - for a patrol to request equipment and QM to check in/out.

 


Notes on forms:

  • You should be able to download (save) these forms onto your computer just by clicking on the highlighted "WFW", "PDF", "RTF", "WP5"or "WPD" and choosing "save file" when your browser asks what to do with the file.
  • For PDF files, the file may display immediately if you've installed the Acrobat Reader plug-in into your browser. Be sure to use the "print" icon on the line immediately above the form to print - do not use "File|Print".
  • If your browser just displays gibberish instead of giving you the choice of downloading the form, try either holding down the shift key as you click, or right-click (click the right mouse button) and select "Save Link".
  • Netscape users (version 3): To set up your browser to automatically download these files when you select them, you can go to "General Preferences"on the "Options" menu, then go to the "Helpers" tag. Click on "Create New Type". Enter the mime type as "application" and the mime subtype as "MSWord" or "Wperfect". Enter "doc", "wp5", or "wpd" as the extension for MS Word or WordPerfect for DOS or Windows, respectively, and click on the "save to disk" choice. Version 4 users, this is available under "Edit|Preferences|Navigator|Applications".
